What is Microsurgical varicocelectomy?

Microsurgical varicocelectomy is a surgical procedure performed to treat varicoceles, which are enlarged veins in the scrotum that can cause pain, discomfort, and infertility. This procedure is considered the gold standard treatment for varicoceles and involves using an operating microscope and microsurgical techniques to carefully dissect and ligate (tie off) the affected veins.

Doctors Info on Microsurgical varicocelectomy

Let’s discuss varicocele for which this surgery is done. A varicocele is a condition characterized by the enlargement of veins within the scrotum. These veins are similar to varicose veins that occur in the legs but are located within the scrotum. Varicoceles most commonly occur on the left side of the scrotum but can occasionally affect both sides. Varicoceles are relatively common and affect approximately 10-15% of males. While some varicoceles may not cause any symptoms and may only be discovered during a routine physical exam, others can lead to symptoms such as:

  • Scrotal discomfort or pain: This discomfort can vary from a dull ache to sharp pain and may worsen with prolonged standing or physical activity.
  • Visible swelling or lump: Varicoceles may be visible or palpable within the scrotum, often described as feeling like a bag of worms.
  • Fertility issues: Varicoceles can interfere with sperm production and quality, leading to male infertility or decreased fertility. They may contribute to decreased sperm count, reduced sperm motility, and abnormal sperm morphology.

Procedure

  • Initial Examination During the initial visit, a local examination is conducted by the surgeon. Additionally, a Doppler study (a type of ultrasound) is incorporated to evaluate the blood flow in the scrotum. A few relevant Blood investigations related to preoperative workup are also performed. The specifics are discussed and it is advised that you schedule an appointment on the day, prior to surgery.
  • Day of the Procedure
    • It is imperative that you remain abstinent from eating or drinking for a minimum of 10 hours prior to the time of surgery. Please bring all your reports with you. The anaesthetist meticulously records your medical history before transferring you to the operating theatre.
  • Steps of the Procedure
    • Incision: A small incision is made in the groin or lower abdomen to access the spermatic cord, which contains the veins affected by the varicocele.
    • Microsurgical dissection: Using an operating microscope, the surgeon carefully dissects the spermatic cord to identify the enlarged veins (varicoceles) and separate them from the surrounding structures.
    • Ligation: Once the varicoceles are identified, the surgeon ligates (ties off) the affected veins using sutures or clips. This prevents blood from flowing backward into the varicoceles, reducing their size and relieving symptoms.
    • Closure: After the varicoceles are ligated, the incision is closed with sutures or surgical staples.
  • Will I be Awake during the procedure? A regional anaesthesia is administered, either via spinal or epidural injections. A needle is inserted into your spinal area, and the drug is then pushed through it. Despite being awake, you will not experience any discomfort below your waist during the ongoing surgical procedure.
  • After the Procedure? You are sent to the recovery room with a dressing in your scrotal area. The vitals are monitored, and the dressing area is thoroughly inspected for any signs of soaking from the surgical site. Dr Ashish will provide an explanation of all post-operative care required, and you will be discharged on the same day. It is recommended that you revisit after two days for the change of dressing and review medication.
Advantages of Microsurgical varicocelectomy

Advantages of Microsurgical varicocelectomy

Microsurgical varicocelectomy offers several advantages over traditional open varicocelectomy techniques, including:
  • Higher success rates: Microsurgical varicocelectomy has been shown to have higher success rates and lower recurrence rates compared to other techniques.
  • Preservation of lymphatics and arteries: The use of an operating microscope allows for precise dissection and ligation of the varicoceles while minimizing damage to surrounding lymphatic vessels and arteries.
  • Reduced risk of complications: Microsurgical techniques reduce the risk of complications such as damage to the spermatic cord, hematoma (collection of blood), and infection.
  • Faster recovery: The minimally invasive nature of microsurgical varicocelectomy typically results in faster recovery times and less postoperative pain compared to open surgery.

Is Microsurgical varicocelectomy Safe?

Yes. Highest success rates with the lowest side effect risks.

When Microsurgical varicocelectomy is needed?

Microsurgical varicocelectomy may be recommended in several situations when a varicocele is causing significant symptoms or complications.
  • Scrotal pain or discomfort
  • Infertility: Varicoceles can interfere with sperm production and quality.
  • Testicular atrophy
  • Adolescents with persistent varicoceles
  • Failed conservative management: If conservative measures such as scrotal support, pain medications, or lifestyle modifications fail to alleviate symptoms

What would disqualify me from Microsurgical varicocelectomy Surgery?

  • Individuals with severe medical conditions that pose a significant risk during surgery
  • Active infections in the scrotal area
  • Severe scrotal pain from other causes: Varicoceles are a common cause of scrotal pain, although not all instances of scrotal pain can be attributed to varicoceles.
