Orthoplasty (a term used in Europe to refer to chordee excision and penile straightening) in patients with epispadias. It is a surgical procedure used to correct the anatomical abnormalities associated with epispadias, a congenital condition where the urethra opens on the upper side of the penis instead of the tip. Repair is generally performed for functional and cosmetic reasons.Doctors Info on Orthoplasty
The decision to proceed with urethroplasty should take into account the patient’s overall health, the severity and complexity of the urethral stricture, and their goals for treatment. Schedule ConsultationProcedure
- Initial Examination
- A comprehensive evaluation is conducted, encompassing a review of the medical history and an exhaustive physical examination. Diagnostic procedures, such as ultrasound, may be employed to ascertain the severity and location of penile curvature.
- Day of the Procedure
- You are not allowed to consume or drink anything for a minimum of 10 hours before your surgery. Please ensure you carry all previous records. An intravenous line is secured to administer antibiotics and fluids. A detailed history will be taken by the anaesthetist before transferring you to the theatre.
- Day of the Procedure
- This procedure is a straightforward office process, with no prior preparation required. A small volume of the patient’s blood is collected, and the plasma is subsequently removed from it in a laboratory procedure known as centrifugation.
- Step Process of the Surgery
- Incision: A small incision is made on the side of the penis opposite to the curvature. The location of the incision may vary based on the specific technique used.
- Measurement and Marking: The surgeon measures the degree of curvature and marks the areas where plication stitches will be placed to straighten the penis.
- Plication Stitches: Non-absorbable sutures or stitches are placed on the longer side of the penis, creating pleats or folds that reduce the length on that side. This effectively straightens the penis.
- Tissue Trim (if necessary): In some cases, excess tissue may be trimmed to further reduce curvature and achieve better symmetry.
- Closure: The incision is closed with sutures or stitches, and a sterile dressing is applied.
- Will I be Awake during the procedure?
- The surgical procedure is typically performed under general anesthesia or spinal anesthesia with sedation, contingent upon the patient’s health condition and the surgeon’s preference.
- After the Procedure
- Upon awakening, you will be transferred to the post-operative room. The dressing in the groin area is monitored for absorption. Subsequently, you will be shifted to your room. A catheter has also been established to ensure proper drainage of urine and facilitate the healing of the urethra.
Advantages of Orthoplasty
- Improved sexual function: The primary benefit of Orthoplasty is achieving a straighter penis, which can significantly improve sexual function. This can lead to:
- Increased ease and enjoyment of sexual intercourse: Reduced curvature can allow for easier penetration and a more comfortable experience for both partners.
- Enhanced confidence: Addressing a physical concern like penile curvature can boost confidence in appearance and sexual performance.
- Reduced pain: In some cases, curvature can cause pain during erections or intercourse. Orthoplasty can alleviate this discomfort.
Side effects of Orthoplasty
- Infection
- Bleeding
- Loss of sensation in the penis
- Shortening of the penis
- Recurrence of curvature
- Erectile dysfunction
What would disqualify me from Orthoplasty Surgery?
- Active infection
- Severe coexisting medical conditions
- Inadequate tissue for reconstruction: Urethroplasty often involves using tissue grafts or flaps to reconstruct the urethra
- Unrealistic expectations: Patients with unrealistic expectations or psychological concerns may not be good candidates for urethroplasty.
- Severe urethral stricture
- Poor overall health: Patients who are in poor overall health or have multiple comorbidities may not be good candidates for urethroplasty.

- Peyronie’s disease: This is a condition that causes scar tissue to form in the penis, which can lead to pain, curvature, and difficulty getting or maintaining an erection.
- Trauma: Injury to the penis can also cause curvature.
